At Gardens Memory Care, we are dedicated to providing the most compassionate and effective support for our residents living with dementia. That’s why our organization proudly employs PAC Certified Independent Professionals who use approved methods and content from Teepa Snow and Positive Approach to Care®.

This world-renowned approach, developed by dementia care expert Teepa Snow, is a cornerstone of our care philosophy. It shifts the focus from “dealing with behaviors” to “creating a positive and caring environment” by recognizing and responding to unmet needs.

What is the Positive Approach to Care (PAC)?

The Positive Approach to Care (PAC) is a person-centered philosophy that focuses on “doing with”, not “doing to”. It helps our care partners develop new skills to connect with residents by emphasizing their retained abilities and promoting a sense of control and self-direction.

Our certified professionals are trained in the core workshops developed by Teepa Snow’s organization. This training provides them with the skills to create positive outcomes, reduce anxiety, and improve the quality of life for our residents.

This model is a powerful tool that helps us understand the different states of being and abilities of those living with dementia. Rather than focusing on a “loss,” the GEMS™ model (Sapphire, Diamond, Emerald, Amber, Ruby, and Pearl) helps our team:

– See the individual first, comparing their characteristics to precious jewels.

– Adjust expectations and communication to match the resident’s current abilities.

– Provide the “just right” care that allows each resident to “shine”.

Positive Physical Approach™ (PPA) and Hand Under Hand™ (HUH)

Positive Physical Approach™ (PPA): Our team learns to use a supportive stance and be aware of personal space to connect with residents in a way that feels safe and supportive, not confrontational.

Hand Under Hand™ (HUH): This gentle technique is used to substitute for a resident’s lost abilities, not subtract from them. By gently guiding “hand-under-hand,” we can assist with mobility, helping the resident feel productive and valued rather than being “done to.”

We are so grateful for the care my mother receives at San Jose Gardens. The memory care staff are compassionate, responsive, and truly know her as an individual. The administrators keep us informed with regular updates, and communication has been excellent. We have also been very impressed with the hospice team, which works seamlessly with the staff to ensure my mother's needs are met promptly and compassionately. Knowing she is surrounded by people who genuinely care has given our family tremendous peace of mind. We highly recommend San Jose Gardens.

My husband was a resident at Ortega Gardens. He had the very best of care. The staff treated both of us with dignity and respect. It is not an easy job caring for someone with dementia but they did it with kindness and skills that I appreciate as a former nurse. I can’t say enough about the great care in a beautiful setting.

The overall patient care at San Jose Garden’s is exceptional. The nursing and caregiving team is attentive, compassionate, and truly treats every resident like family. They creat a personalized care plan for each resident focusing on mobility challenges and dementia while still encouraging independence. The medication management is also out standing. They have great communication between the family and providers in the facility. They also keep a close eye on nutrition.

I highly recommend San Jose Gardens and the incredible team led by Gwen. They have helped me assist many families whose loved ones are living with dementia and other cognitive impairments, and they consistently provide compassionate care, excellent communication, and a welcoming environment. One thing that always stands out is how well-maintained the community is. Although it is nearly 10 years old, it still feels fresh, modern, and beautifully cared for. The courtyard is especially lovely and provides a peaceful outdoor space for residents and their families to enjoy. The food is also excellent, and the staff goes above and beyond to ensure residents feel supported and engaged. Gwen and her team are true professionals who genuinely care about the people they serve. If you are looking for a memory care community for a loved one, San Jose Gardens is absolutely worth considering.

I’ve worked with the staff at SJG for several years, and they continue to impress me. The building is clean, beautifully remodeled, and truly feels like a welcoming place. 2 years ago we placed my brother in memory care at Ortega Gardens in Jacksonville Fl. It was a very emotional and trying time for our family. Brian Sheridan’s guidance was a huge help to us and definitely a blessing. Every time I step into Ortega I am impressed with the professional, friendly and very respectful atmosphere. My brother is very content and feels right at home here. The staff are here to help and continue to do an excellent job with the residents, family member’s, and visitors. If we have any questions they are excellent at responding quickly. I am very impressed with the staff and how they maintain their day to day. I’m happy to see that my brother is happy and comfortable here. A big thank you to the owners and the staff. You are doing a great job!!

Although I am nearing the end of my loved one's alzheimer's journey, I would be remiss if I didn't take a moment to extol the many virtues of Mount Pleasant Gardens. From my initial interaction with Leanne, to nearly every subsequent one with all manor of staff from Nurse Judy to the many extremely capable, warm, attentive aides, I have been so impressed and comforted by their dedication to their residents. These people are true heros and I know my loved one has received the best care available. I recommend them highly should the time come that you too need a safe, secure, comfortable environment with a very helpful and engaged staff for your loved one.

My mom was sent here after a traumatic accident that damaged her back and brain. San Jose Gardens Memory Care was very supportive, kind and caring. San Jose Gardens is truly a safe place for those with memory care issues. They have excellent upper management in place with many years of experience. Both Gwen and Kathleen keep a sharp eye on any needs that come up with their residents. They are quick to respond to solve any issue, therefore, keeping the residents healthy and happy. You can feel confident placing your loved one in their care.

My mom has lived in three different memory care homes in the Charleston area, and, by far, Mt. Pleasant Gardens is the most welcoming, supportive, reliable, and professional — both for their residents and families. From the moment my mom and I toured with Leanne, MPG’s resident-focused philosophy has driven all decisions regarding Mom’s care. There is detailed and timely communication among the nurse, Judy (whose cell phone number is shared with families), me, and Mom’s doctor. Additionally, families receive emails inviting them to various special events, including the annual fundraiser held to support Alzheimer's research, an event that offers residents an opportunity to run sales and serve their family members. (Leanne is great at sending photos of our lived ones!) This is just one way MPG supports the autonomy and independence of its residents. All activities and interactions among staff and residents reflect the “dignity for all” approach that is practiced every day. As Mom’s Alzheimer’s disease progresses, I am grateful for the MUSC psychiatrist who meets with residents on site and collaborates with Judy and other caregivers; her support of Mom and me is priceless. Finally, I find the public areas as well as Mom’s apartment/room to be the cleanest of all our experiences to date. Whether it is the smile at the front door or the sparkling clean floors that greet me when I arrive, each visit is a positive experience. Expect an open door policy with the director Kristen, the bookkeeper Jennifer, the nurse Judy, and Leanne in sales and all things resident related! This is a team that has been in place for years and is heavily invested in MPG as if their own family members lived there. Alzheimer’s is a difficult journey, and no one understands this better than MPG. I am glad that they walk by my side and Mom’s each step of the way.

Both my parents have lived at Mount Pleasant Garden for the past three and a half years. What I like most about MPG is that staff always know what's going in with the residents, from the front desk people to the marketing people to the Director. Whenever we visit, everyone is always very welcoming and knows exactly how my parents are doing. The facility is very clean and well-maintained and the rooms are spacious. The enclosed garden with covered terraces in the center of the building is a lovely spot for residents to enjoy the nice weather and get some fresh air. They have a private dining room and sitting room that we can book for visits and meals with family, which is a nice amenity. My parents don't participate in much, but there are tons of in-house activities for the residents. They also organize outings to local restaurants. It's definitely the right spot for my parents.

San Jose Gardens has the professional administrative staff and dedicated employees who did their best to take care of our mother. The building exhibits a modern, clean and well maintained appearance throughout, not just the lobby. It is much newer than most places we have visited and the mainteance staff corrects problems in short order. All the rooms are clean and well decorated for the occasions. There are plenty of programs in which the residents may participate every week– musical performances and art classes. We are grateful our mother spent her final years there under their care.

Last year our family diligently looked at and visited many assisted living homes with some type of memory care for my mother. Some not very good.  Thankfully, we came upon Ortega Gardens memory care and what a blessing it was. They are specialized in working and caring for dementia patients. Equally, all of their staff are trained on the compassionate care that was needed for them. Their facility is set up with three identical neighborhoods. From the least affected patients to the ones who progressively go thru dementia and need more care. This way they live and interact with the residents that are in about the same similar condition. It's an outstanding model that more long term care facilities should adopt. Unfortunately, most of the other place we visited lumped all of the dementia patients together. It was sad and not conducive to their disease and certainly not compasionate. The family would sincerely like to thank Ortega Gardens and all of their staff for the care and compassion they showed our Mom throughout her stay and most importantly in her final days.

I highly recommend San Jose Gardens for your loved one. We moved my father there in April.. From the moment we walked in, we were impressed by the warm, welcoming atmosphere and the professionalism of the staff. The caregivers are attentive and genuinely care about the well-being of the residents. My father has adjusted well and is enjoying the various activities and social opportunities they offer, one of the nurses even plays chess (dad's favorite activity) with him :). The facility is clean, well-maintained, and the staff is easy to contact and responsive to our questions. We feel at ease knowing that my father is in a safe and nurturing environment.

Ortega Gardens (on Timuquana Road) saved my life. My mother had rapidly progressed and needed 24 hour assistance. My siblings and I tried to take care of her in each of our homes. We had to keep passing her back and forth and that wasn't healthy for any of us, especially my mother. We tried the adult daycare route. That was not much easier. She absolutely needed more care than we could provide. It was impossible to work and take care of her at the same time and I financially couldn't retire. Ortega Gardens has been able to take care of her activities of daily living as well as her doctor appointments. My mother has been a resident of Ortega Gardens for almost two years and they've done a better job than I could've. I'm thankful to have found them.

San Jose Gardens was highly recommended to me for my mother as her dementia became more advanced. It's a scary time for your loved one with dementia to enter into a new living environment but SJG has such an intimate atmosphere and staff with smiling faces whose care makes a world of difference. I also see a lot of teamwork among the staff which helps to make the day run as smooth as possible for each resident. I know my mother is properly cared for at SJG and for that I am greatful!

Only been here through agency. But I must commend this facility staffing. It's like they carefully handpicked their workers. They are such team players and so nice to each other and the residents. I've never been at a facility before that made me feel like I was apart of their team like this one. Even the lady at the front desk is super nice and helpful and welcoming. This is the kind of work environment that everyone should be able to work in. Keep doing what you're doing to make this place great for seniors.

My mom was placed in San Jose Gardens roughly two months ago. She had been at 2 other places prior, but I have never seen her as happy as she is now. The Staff and Care Givers are wonderful with her and I sleep well knowing that she is safe and well taken care of.

As a professional care manager who has had several clients here I am amazed at the staff and their ability to manage complex residents. I appreciate how the executive director and the director of nursing think out of the box for solutions! But their compassion is where they really shine! What a blessing PVG has been for my mother the past 2.5 years! She is well cared for and the amazing activity director, Dee, keeps her and the other residents engaged and active with high quality, creative activities and events. My mom loves to be able to go on the wonderful outings to the beach, cat cafe, ice cream shop and more. I know it takes care and planning as well as partnerships with local businesses to make this possible. I am beyond grateful that my mom is treated with the kindness and dignity she deserves.
